Child Death Review Board members

The Board has 12 members, each appointed by the Minister. The current Chair is the Principal Commissioner of the QFCC. Other members are appointed for a period of no more than three years but may be reappointed.

To reflect the social and cultural diversity of Queensland, the Board must:

  • include at least one Aboriginal or Torres Strait Islander member 
  • include persons with relevant experience, knowledge and skills 
  • include at least 50% non-Government members.

Experts may also be invited to work with the Board to deliver its functions.

The Board is supported by a secretariat that is hosted within the QFCC.

Mr Luke Twyford

Chairperson 

Luke, in his position of Principal Commissioner of the Queensland Family and Child Commission, was appointed as the Chairperson of the Child Death Review Board on 17 March 2022. Luke is an accomplished senior executive, with rich experience in the child and family sector.

Ms Simone Jackson

Deputy Chairperson

Simone Jackson is a proud Kamilaroi woman from southwest Queensland and an accomplished Government Executive with over 20 years’ experience as a public servant. 

Detective Superintendent Denzil Clark

Member

Detective Superintendent Denzil Clark commenced with the Queensland Police Service (QPS) in January 1988 and has served the past 33 years as a detective in various positions across the QPS. 

Dr Meegan Crawford

Member

Dr Meegan Crawford is the Chief Practitioner for the Department of Families, Seniors, Disability Services and Child Safety. 

Dr Divna Haslam

Member

Dr Haslam is a registered clinical psychologist with a PhD in the field of parenting.

Mr Darren Hegarty

Member

Darren Hegarty is the Assistant Chief Operating Officer and the Youth Justice representative on the Board. 

Ms Carly Jacobitz

Member

Carly Jacobitz is the current Executive Director – Performance, Risk and Assurance (Child, Youth and Family) at Life Without Barriers (LWB). 

Ms Beth McNamara

Member

Beth McNamara is a qualified social worker (First Class Honours, University of Sydney) who lives and works on beautiful Gubbi Gubbi/Kabi Kabi country.

Dr Stephen Stathis

Member

Dr Stephen Stathis obtained a dual fellowship in paediatrics and psychiatry, with certificates in Child & Adolescent Psychiatry and Forensic Psychiatry. Stephen is currently the Medical Director of Child and Youth Mental Health Services, Children’s Health Queensland. 

Member

Murray Benton is a proud Aboriginal Barkindji Koori man from Central West New South Wales. He is the Deputy CEO of Youth Justice with the Queensland Aboriginal and Torres Strait Islander Child Protection Peak. Murray brings extensive experience across child, youth, and family services.
